I remember seeing pictures of a piston and con rod embedded in the track after departing a British Railways Peak class diesel. It turned out a fitter had fitted the wrong size fuel injector nozzle to one cylinder and on a full power climb of Shap the combustion pressure exceeded design limits. Oops.

I remember seeing pictures of a piston and con rod embedded in the track...
One of the replies to that image mentioned a Deltic loco throwing a rod out through the side and killing a cow.
